Provide mobility assistance:
Support patients in safely moving in and out of beds, baths, wheelchairs, or vehicles.
Maintain a comfortable living environment:
Assist with housekeeping tasks such as changing bed linens, doing laundry, cleaning the home, and helping with personal care needs.
Document and communicate patient progress:
Keep detailed records of patient care, health conditions, and any observed changes to ensure continuity of care and effective communication with healthcare professionals.
Engage patients in mental stimulation:
Foster meaningful interactions with patients through conversations, reading aloud, and other activities to promote mental well-being and cognitive engagement.
Administer medication reminders:
Ensure patients take their medications as prescribed by providing timely reminders.
Encourage physical activity:
Graduated from an accredited Home Health Aide program
High School Diploma or GE
Driver’s license required
CPR certification preferred
2. In Home Assessment: During the assessment, we will ask questions that will assist us with your individualized care plan, answer any additional questions, and complete paperwork.
3. Start of Care: At the start of care, a supervisor will come with the Caregiver and present the care plan and assist with introductions.
4. Client Visits: Once care has begun, we will continue to ensure you are providing exceptional care and adjust care plan if needed.
